Contact form generator

Choose the fields, then copy the HTML into your page. On a Pagehaus site the form works as it is: messages are emailed to you, with no PHP and no form service to sign up for.

Using it

On Pagehaus: paste it into any page. Messages arrive by email and are kept on your site's Forms tab; replying to the email replies to the visitor. See contact forms for more options, such as sending messages to a client's address.
Elsewhere: the HTML is standard, but /_form/… only works on Pagehaus. On another host, change action to the address your form service gives you.
